ALL patients on clopidogrel must have a “Clopidogrel indications note” – you must state the reason why they are on clopidogrel and the duration of therapy
1. Go to Notes (bottom tab)-> New Note (bottom left side of screen) -> Clopidogrel Indications Note
2. The note will then prompt you for the reason (ACS, CABG, Stent (single/multiple/left main/etc.) and duration of therapy
3. Then you can order the medication for one year at a time
*If you are unsure how long the patient should be on Plavix you can use a cardiology or neurology E consultation for them to review the outside records and give you a recommendation based on the reason it was started in the first place (sometimes the discharge summary from OSH does not clarify how long they need to remain on Plavix)
*You do not need to do a clopidogrel indications note for refills as long as the original note indicates they are on extended therapy i.e. lifelong clopidogrel for left main stent. You do however need to do a new note if they had 12 months of therapy which they completed then had a repeat heart attack then needed another 12 months for example. (basically, if the medication is being renewed for an old indication you do not need a new note. If there is a new indication with a new duration of therapy, you need a new note)
